You can pre-register your child’s details for our Elim Kids Sunday programmes by completing the form below.
In order for us to care for your child to the best of our abilities we need some information from you, a Parent or guardian. Our check-in form asks things like what allergies your child may have, how many children are in your family, and get your contact information.
This can take extra time on your first Sunday. By completing this form at least 72 hours prior to the Sunday service we can have your child’s details ready for self check-in.
Please note, this is a one time form. You will not need to complete this information again for your child.
